ENTRY FORMS,SCORE SHEETS, DISKS AND FEES

All City DiveMeet 2004

Itis your responsibility to complete and deliver entry forms to the Maple Bluffpool by 6:00 p.m. July 19^th.   At that time you will receivescore sheets for each diver.

Theentry sheets must include the diver’s name and birth date. There is also a separate page for eachevent and a summary sheet of all of your entries.

Acomputer disk with a list of your divers and their ages should also besubmitted. Please use IBMformatted Microsoft Excel Software. Also, please make a separate file for each sex and age group.

Youmust include a check made out to Maple Bluff Country Club for the number ofentries @$3.oo per diver.

Note:  All eligible divers must have competedin a minimum of three dual meets during the summer season and must be aresident of Dane County.
