ALL CITY DIVE MEET 2005

It is your responsibility to complete anddeliver entry forms to the Monona Pool by 6:00p.m. july 18^th. At that time you will receive score sheets for eachdiver.

The entry sheets must include the diver’s name and birthdate. There is also a separate page for each event and a summary sheet of allof your entries.

A computer disk with a list of your divers and their agesshould also be submitted. Please use IBM formatted Microsoft Excel software.Also, please make a separate file for each sex and age group.

You must include a check made out to Monona Swim and DiveClub for the number of entries @ $5.00 per diver.

Note: Alleligible divers must have competed in a minimum of three dual meets during thesummer season and must be a resident of Dane County.
