ENTRY FORMS, SCORE SHEETS, DISKS AND FEES

 

It is the responsibility of each coach to complete and deliver entry forms to the Nakoma Pool by 6pm, July, 16^th.  At that time you will receive score sheets for each diver.

  

The entry sheets must include the diver’s name and birth date.  There is also a separate page for each event and a summary sheet of all of your entries. 

 

A computer disk with a list of your divers and their ages should also be submitted.  Please use IBM formatted Microsoft Excel or Microsoft Word Software or a similar file format.  Also, please make a separate file for each sex and age group.

 

You must include a check made out to Nakoma Country Club for the number or enties @$3.oo per diver.

 

Note:  All eligible divers will have competed in a minimum of three dual meets during the summer season and must be a resident of Dane County.
