ENTRY FORMS, SCORE SHEETS, DISKS AND FEES

2003 All City Dive Meet

 

Itis your responsibility to complete and deliver entry forms to the Seminole poolby 6:00 p.m. July 14^th. At that time you will receivescore sheets for each diver.

Theentry sheets must include the diver’s name and birth date. There is also aseparate page for each event and a summary sheet of all of your entries.

 

Acomputer disk with a list of your divers and their ages should also besubmitted. Please use IBM formatted Microsoft Excel Software. Also, pleasemake a separate file for each sex and age group.

 

Youmust include a check made out to Seminole pool and tennis club for the number ofentries @$3.oo per diver.

 

Note: All eligible divers musthave competed in a minimum of three dual meets during the summer season andmust be a resident of Dane County.
